That Benefit Form We are All Looking For!
At the 20 January 2010 Labour-Management Meeting of 1356 the following information was confirmed, the form exists!
The form that is to be used to assign the benefit to a provider, like the visit to get orthotics and you use the form and the orthotic shop gets the payment direct from Sunlife can be found at our website http://1356.cupe.ca/www/health-care-benefits-form and the direct link to the form.
